Medication Management Resources

Throughout the treatment of Alzheimer’s or other dementia one may be prescribed a variety of medications. These powerful medications can be used for the symptoms of the disease itself as well as additional conditions. Care must be taken when one requires multiple medications at different points throughout the day. 

With the frequency and power of a patients prescribed medications, the organization and coordination of their care is essential. The Alzheimer’s Association (ALZ.org) has complied a list of safety tips to successfully coordinate and document pharmaceutical care to prevent any medication related problems. 

It is important to make sure that the doctor(s), pharmacist, and caregiver are all on the same page. The Alzheimer Association recommends keeping a log of current prescribed medications, over-the-counter medications, and also all vitamin or herbal supplements. Be sure to include each respective dosages in the log as well. This log should be brought to each of a persons appointments ensure proper coordination between all parties.

This log will also allow you to double check any new prescriptions with the pharmacist or doctor for potential interactions with current medication. As well as record any side affects or allergies that have occurred in the patients medical history. 

Within this powerful log it is also important to record as much information as possible about each prescribed medicine. The Alzheimer Association recommends recording each medication’s name, purpose, dosage, frequency, and possible side effects. 

Once the proper dosage and frequency of the new medication is determined it is imperative to follow their instructions. Any change in this can lead to unforeseen side affects or interactions.
